Source code in a developer's personal account does not really belong to the company. The person leaves and access to the whole change history goes with them. Moving the repositories to the organisation is the first thing to do, especially as it is free and takes one evening.

Questions and answers

No. External services run more reliably than most self-hosted installations and need no maintenance. Your own environment is needed where the code cannot leave under contract or where independence from foreign platforms matters.

Usually per developer, on an annual subscription. The important thing is to register them to the company. A subscription tied to an employee's personal email leaves with them when they go, and the money is wasted.

Only when that requirement has actually been placed on you. An ordinary commercial team works mostly on free tools, and what has to be bought is mainly the development environments.
